Description: Salud Para La Gente (SALUD) provides high quality, comprehensive and cost-effective healthcare to underserved low-income communities in the Monterey Bay area, including Santa Cruz County and North Monterey County. We began in Watsonville 1978 as a storefront “free clinic,” and in 1992 became a federally qualified health center (FQHC). Today we are a primary health care network with 11 clinic sites, over 400 employees and continue to provide high quality services to patients of all ages.
Under the supervision of the Director of Behavioral Health, the Adult High Risk Case Manager provides direct case management services and care to Salud’s psychiatric and other high risk adult patients.
The Adult High Risk Case Manager is responsible for evaluating, developing, and implementing appropriate case management plans which take into account the complex psychosocial and medical needs of patients and their families, and functions as a key member of the interdisciplinary Intensive Case Management team.

DUTIES & RESPONSIBILITIES:
- Reviews adult psychiatry referrals and determines if the patients will see in-house psychiatry, telehealth psychiatry, or referred to County Behavioral Health based on a review of records and EHR referrals. Works closely with care coordinators and other staff who are helping with specialty services to provide close tracking and case management of our highest risk adult psychiatry patients.
- Contacts patients within a set period and describes psychiatry services and confirms interest in the service. Schedules initial and follow up appointments for both Behavioral Health provider and Psychiatrist(s) and other designated medical psychiatric providers.
- Determines patients' insurance eligibility and assists patients in accessing care that they qualify for. Facilitates referrals to Community Health Services for uninsured patients.
- Collaborates closely with Salud’s Psychiatrist(s) and other designated medical psychiatric providers to ensure patients are attending their scheduled appointments and have appropriate follow-up appointments scheduled. Follows instructions of the psychiatry team when facilitating a transition to a higher level of care or hospitalization. Documents all steps in the EHR.
- Assists the psychiatry team with sending and obtaining records when patients are sent on a hold or transferred between levels of care and/or from hospitals.
- Contacts psychiatric and high risk adult patients by phone, secure text message, or other means when they have missed an appointment, or to relay information to the patient from one of the team members. Makes multiple attempts to reach patients who have missed appointments and documents these attempts.
- Maintains patients' records by reviewing case notes; logging events and progress effectively and efficiently in the electronic health record (EHR). Monitors patient’s treatment compliance and referral outcomes.
- Communicates patients' progress by conducting or attending interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and patient’s family; identifying treatment influences.
- Prepares patients' successful termination from program by reviewing and amplifying termination and treatment plans; coordinating treatment plan requirements and referrals to community resources; orienting and training family members; providing resources and notifying other providers on the health team when treatment goals are met.
- Improves staff competence by providing educational resources; balancing work
- requirements with learning opportunities; evaluating the application of learning to changes in
- treatment results. May provide trainings to community partners and/or internally.
- Improves treatment results by studying, evaluating, and re-designing processes;
- implementing changes; assisting in rewriting policies and procedures as appropriate in conjunction with the Director of Behavioral health and /or Psychiatry team.
- Prepares reports by collecting, analyzing, and summarizing treatment and results data and
- trends; compiling statistics; completing grant and subsidy applications as needed. Manages databases relevant to position and uses data to assist in required reporting.
- Attends team meetings and other relevant community collaborative meetings with approval from supervisor to maintain strong community connections to ensure best care and service provision to our patients.
- Coordinates with other team members to ensure linkage with appropriate services for each patient including primary care providers and clinical supervisor, mental health services including screenings, counseling, referrals, treatment plans and ongoing assessments in accordance with SPLG expected standards.
- Ensures compliance with regulatory bodies and in-house clinical guidelines
- Develops care plans for patients and provides therapeutic support as needed
- Adheres to the standards and policies of the Organizational Privacy/Security and
- Compliance Programs, including the duty to comply with applicable laws and regulations
- (HIPAA, OSHA, OIG, guidelines, and other State and Federal laws). This also includes
- reporting to the Board of Directors, Compliance Officer, Privacy Officer, or supervisor any
- suspected unethical, fraudulent, or unlawful acts or practices.
- Consistently adheres to and/or exceed SALUD’s communication guidelines and expectations with internal and external customers including co-workers, patients, and external partners.
- Other duties and projects as assigned
Requirements: MINIMUM QUALIFICATIONS:
- Master’s Degree in Social Work or related field or Bachelor’s degree with equivalent experience
- Ability to move between sites and perform duties in the field in a variety of settings, use of own vehicle to do so
- General understanding of common medical and psychological disorders especially of adults.
- General understanding of the complex needs of families in our community
- Maintains an updated CPR certificate, valid California driver's license
- Experience working in a community health center setting or similar environment is desired.
- Fluent Spanish/English Required
- Knowledge of or reasonable ability to learn effective use of Electronic Health Record
PHYSICAL DEMANDS:

SALARY AND BENEFITS:
Salary: $64,471 - $74,625/year
