ASSISTANT CLINICAL DIRECTOR—HOME HEALTH & HOSPICE

CLINICAL DIRECTOR ESSENTIAL FUNCTIONS
-Provides leadership and direction
-Quality Assurance and Program improvement- PIP Improvement plans.
-Staff development and on boarding training.
-Prepares notes/reports for Care Conference and works within a team to provide best practice care
- Oversees clinical team, patient care, quality assurance...
-Creates 485s/Plans of Care
-Approves all orders in workflow
-Conducts care conference
-Assures ongoing coordination of care among all disciplines touching the patient/family
-Performs patient visits as needed
-Interfaces with community partners, families, and various referral sources
CLINICAL DIRECTOR QUALIFICATIONS
-Cheerful customer service oriented.
-Current Utah RN license (required).
-Management experience preferred.
-Experience in hospice and home health settings.
-Demonstrated ability to triage and apply critical thinking skills.
-Knowledge of and ability to navigate electronic medical records. MatrixCare experience a plus.
-Reliable transportation is required.

- Nursing: 3 years (Preferred)
