The Behavioral Health Case Manager ECM: provides case management and coordination of services to ECM eligible target populations. The case manager is a part of a collaborative team working to provide stability and resources to individuals living with a variety of complex physical health circumstances. The case manager facilitates communication between the patient, mobile med team, behavioral team and other CommuniCare departments and outside agencies. CommuniCare Health Centers is a Federally Qualified Health Center providing comprehensive patient-centered primary care services including medical, dental, substance abuse treatment, behavioral health, and outreach services to the low income, multi-ethnic, uninsured and underinsured in the communities we serve.

SCHEDULE: Full time (40hr/week, some evening hours required) in Woodland, California. Some travel in Yolo County will be required.

PAY RATE:

  • The pay range for this position is $22.75/hr.-$32.05/hr. Starting pay within this range will vary depending on the job-related knowledge, skills, and experience.
  • May be eligible for annual 2% + 0-2% merit-based pay adjustment (0-4%).

RESPONSIBILITIES:

  • Provide care coordination to clients with multiple medical needs and/or those determined to be at high risk for complications.
  • Effectively communicate with both patients and providers in person, via telephone or email.
  • Accompany patients to appointments with providers and in the community as needed to support identified needs.
  • Obtain and process Enhanced Care Management referrals from agency programs.
  • Communicate immediate health issues to the patients medical provider or care team.
  • Comply with all Enhanced Care Management operating procedures and reporting requirements.
  • Work with all members of the care team and other departmental staff to expedite proactive solutions to issues, serving as primary patient contact for issues concerning care access and service.
  • Provides a consistent source of encouragement, hope, and non-judgmental support to patients with a variety of challenging circumstances.
  • Participates in staff meetings. Assists with coordination of care offering input on patient progress.
  • Maintain clerical duties: medical record charting & patient tracking forms.
  • Responsible for remaining within established ethical standards, including those of confidentiality.
  • Other duties as assigned.

MIMUM POSITION REQUIREMENTS:

Education: Bachelors in social work or other related field recommended, but not required. Experience providing direct services to patients as a case manager or other member of the health care team.

Knowledge/Experience: Experience working in a social service, health related organization or medical office/clinic. Excellent interpersonal, oral and written communication, and presentation skills. Strong organizational skills and the ability to multitask. Demonstrated ability to assume responsibility and to work independently as well as in a team. Ability to work in a busy environment. Knowledge of medical terminology strongly preferred. A service orientation characterized by cultural sensitivity and polite communication with patients and staff members.

Special Skills/Equipment: Must provide proof of current drivers license & auto insurance.
