These include consultation and service provisions to area schools, other community agencies, groups and individuals, as well as other cooperative prevention efforts.
Job Summary:
- Provide targeted mental health case management for youth with serious emotional disturbances and co-occurring disorders
- Meet with enrolled and potential clients and their families to create a Plan of Care
- Conduct comprehensive assessments and other assessments as required by DHMH
- Coordinate and facilitate Family Team Meetings
- Provide management of the POCs
- Collect information during the application process and as identified in each POC
- Identify providers, supports, and resources for clients· Coordinate meetings with client and family to (a) meet with providers to ensure goodness of fit for proposed services and products in compliance with the POC (b) meet with family peer-to- peer support, intensive in-home service, mobile crisis response service, and other providers appropriate to the POC
- Maintain client clinical documentation using EHR
- Provide on-call services as scheduled
- Promptly respond to client crises
- Provide case management for client open-access as scheduled.
- Facilitate groups
Minimum Qualifications:
- Bachelor’s degree preferred; or High School Diploma or its equivalent and 21 years or older, was a participant in, was a direct caregiver, or is a direct care giver for a person(s) who received child and family services from the public, child and family services system.
- Ability to observe and accurately report the functioning of children and families.
- Ability to handle stressful situations.
- Must maintain a flexible work schedule to meet scheduling needs of clients
- Must meet Care Coordinator training and certification requirements established by DHMH
- Must complete trainings required by CSA and BHA, including CASII, ESCII, and CANS
- Must complete Wraparound certification.
Job Type: Full-time
Pay: $37,000.00 - $42,000.00 per year
