Program Overview:
The Dale Jones Burch Neighborhood Center is a free walk-in program that provides case management and short-term crisis counseling services, benefit/entitlements screenings and referrals, health insurance enrollment services and parenting classes/workshops, and with a number of field-based community health worker initiatives. Services are offered in English, Spanish and Chinese.
Qualifications:
- BSW or BA/BS college degree in human services related field; 1-2 years case management experience preferred. Will consider entry-level/new grads and also welcome MSW candidates/MSW graduates.
- Interviewing, assessment and engagement skills; empathy for low-income/vulnerable, culturally diverse, physically/mentally disabled and/or elderly populations; ability to work collaboratively with case management team and/or independently on- or off-site as needed to deliver case management services in a comprehensive and timely manner.
- Familiarity with casework/human services practices/principles/code(s) of ethics.
- Knowledge of range of public benefits (SNAP/Food Stamps, Access-a-Ride, Reduced Fare Metro Card, Medicare Savings Program, Medicaid Spend Down, NYCHA/Section-8/public/affordable housing/lotteries, Medicaid/Medicare, SSI/SSD/SSA, etc.) and eligibility criteria and/or a quick learner willing to attend trainings, and apply concepts/eligibility criteria when screening households, and ask questions as needed
- Solid writing/grammar/typing and organizational/time management skills; diligence when completing accurate and timely case documentation; basic math/calculation skills for conducting financial assessments of households.
- Familiarity with record keeping and data entry required for reporting/tracking purposes as per private program funding and quarterly/annual contract goals.
- Bilingual in English/Spanish required
Responsibilities:
- Provide case management services including biopsychosocial needs assessments and crisis intervention to adults 18 and over; screen households for benefits and offer referrals as needed for legal/financial counseling. mental health counseling, workforce development, senior services, health insurance, parenting/family care-planning needs and more.
- Conduct benefit screenings and prepare/submit applications via designated channels (ACCESS HRA and other portals) and track outcomes in Foothold Awards and/or Benefits Data Plus database.
- Maintain regular case management documentation records (date/time/intervention and follow- up).
- Advocate on behalf of clients; prepare and submit requests for emergency financial assistance to funding partners with assistance from case management team (United Way, private funders).
- Participate in weekly Case Review Team Meetings either via Zoom online or in-person, when staff return onsite (TBD) and 1-1 supervisory meetings with task supervisor; proactively participate in interdisciplinary case management team approach together with other case managers, social workers, community health workers, task supervisor, and NRC Program Director to identify and meet client needs; receptive to constructive guidance and supervision; i.e., “learning by doing” regarding comprehensiveness and accuracy of casework activities and follow-up.

Essential Physical Job Functions:
- Able to walk up and down flights of stairs at the NRC and/or Dale Jones Burch Neighborhood Center.
- When HSS/NRC staff return on site, be willing to utilize PPE/Personal Protective Equipment (face mask, hand sanitizer) and engage in social distancing when serving clients for safety reasons.
- Fully vaccinated against COVID-19
- Ability to work onsite in the Lower East Side

Required- Bachelor Degree or better
