CLINICAL SUPPORT SPECIALIST - MANAGED CARE PLANNING Job at Cape Fear Valley Health System

Position Summary:
The primary function of this position is to provide administrative and clinical support to care navigation team. The medical assistant can expect to assist with documentation, telephonic education, assessments and continuation of patient care .
Knowledge, Skills, and Abilities:
One to two years’ experience in a clinic with geriatric and/or community health care experience preferred. Must have excellent written and verbal communication skills. Must be able to work independently, setting priorities to coordinate care plan efficiently as well as be able to work effectively in a team environment under supervision. Must be able to effectively communicate with patients and their families and members of the patient care team inside the organization. Must have some experience with behavioral and educational strategies including, but not limited to motivational interviewing, teach-back method and self-management support. Must have the ability to adapt quickly to changing patient needs and applicable organizational and federal regulations.
Required Education, Licenses, and Certifications:
- Diploma or Certificate Program Nursing Assistance
- CERTIFIED MEDICAL ASSISTANT
- CERTIFIED NURSING ASSISTANT I

Other Information:
Some light carrying and lifting may be required. Occasional walking may be required to access all areas of the Medical Center. Ability to effectively communicate orally to patients, family members, personnel and physicians. Near visual acuity to proofread hand or typewritten materials. Manual ability to use telephones and computer keyboard.
