Catholic Charities of Southern Minnesota seeks a full-time Community Connector/Community Health Worker in our Winona Office. Position employs community-based care coordination strategies outlined in the Pathways Community HUB model to improve health outcomes for targeted populations. This position will provide support to at-risk clients as they navigate the health care and social service systems in the Winona area community which aims to increase access to support services, improve the quality and cultural competence of their care, and increase their health knowledge and self-management skills of underserved populations. This position requires extensive travelling throughout the Winona community working with varied populations, oftentimes in client homes.

Specific Duties and Responsibilities:

  • Serves as a critical linkage between health and social services agencies by providing compassionate and professional outreach to at-risk clients as identified by the HUB.
  • Follows-up with clients in their homes or other community-based locations to conduct in-depth evaluation of their social, medical, employment, housing, financial or other social needs.
  • Applies their shared life experience and training to assist clients with education, advocacy, care coordination, coaching and navigation in order to ensure these needs are systematically addressed.
  • Provides communication support and acts as a client advocate on issues of access and use of medical, social or other services.
  • May facilitate transportation for clients to access healthcare or social service appointments.
  • May attend appointments with clients, facilitate paperwork completion, ensure instructions are understood and that progress towards self-sufficiency is being achieved.
  • Ensures the privacy and security of Protected Health Information (PHI) and procedures are HIPAA compliant.
  • Documents client interactions in a timely and thorough manner using a HIPAA-compliant electronic data collection platform.
  • Attend meetings and trainings as needed, including participation in community of practice meetings monthly. Overnight travel to conferences or community health worker trainings may be required on occasion.
  • Distribute and discuss health promotion information and community resource information to community groups as assigned.
  • Works in partnership with the Live Well Winona (LWW) pathways Winona Community HUB staff, area health care organizations, social service organizations and other partners.
  • Requires extensive travel throughout the Winona area community.

Qualifications:

  • High school graduate or equivalent; some college preferred. Preference for Community Health Worker (CHW) certification. Employment contingent upon achievement of CHW certification within two years of hire; the certification program is at the discretion of Catholic Charities’ Housing Program leadership. Upon hire and completion of CHW certification be able to demonstrate an understanding of CHW health care delivery system and the Pathways Community HUB model.
  • Have a deep knowledge and understanding of the Winona area community and is uniquely equipped to bridge barriers related to culture, race, language, literacy, socioeconomic status or other factors.
  • Intermediate computer and phone skills. Ability to use e-mail and navigate the internet, and perform data entry in a timely manner. Ability to communicate effectively orally and in writing.

  • Must demonstrate maturity, flexibility, creativity, initiative, organizational skills, the ability to work effectively with others, and the ability to think systematically.
  • Ability to communicate, both in writing and verbally, information in a direct straightforward manner without excessive use of technical terms.

  • Must own vehicle and possess valid driver’s license and proof of insurance.
