Status/time: Full-time (40 hours/week)

Hourly rate: $20.00/hour Position Summary: This is a full-time, temporary, contract position funded for one year.

The Community Health Worker (CHW) will conduct COVID Vaccine Hesitancy community assessments, outreach and health education around COVID-19 and act as a bridge to COVID-19 testing, vaccine, and support services. This position is based in South and West Suburban Cook County and include some night and weekend work.

Responsibilities

· Under the direction of the Associate Director of the Community Assessment and Engagement COVID Community Response Services, and in collaboration with the mobile health wellness services team the Community Health Worker (CHW) will provide ongoing COVID 19 vaccination outreach, education, and increased access to care to enhance client/patient connectivity to available treatment resources

· The Community Health Worker helps to increase acceptance of COVID-19 vaccination in disproportionately affected African American communities.

· Addresses the specific and unique needs of youth, parents, seniors, and immune compromised individuals to increase vaccine acceptance.

· In collaboration with the mobile health wellness services team, recognizes the needs of the community and identifies strategies/interventions to improve vaccine confidence and access, and works with leadership team to implement strategies/interventions.

· Connect with patients in the community, as well as via social media

· Engages individuals in health education/promotion events and services offered by Alliance Care 360 through the program

· In collaboration with other outreach and care coordination personnel and efforts, conducts outreach to community members and other organizations in the South and West Chicago suburbs regarding vaccine hesitancy acceptance amongst substance misusers, shelter homeless and immune compromised individuals.

· Review the IDPH EVA Dashboard screening tools and connect with clients who screen as potentially benefitting from community linkage to services.

· Linkage to care: Facilitate referrals to and assessing of services at culturally responsive healthcare providers and systems of care; help participants succeed in overcoming barriers to making appointments and then keeping them (follow-up, coordination of mobility, etc.)

· Events Management: The CHW will have partner with Health fair organizers in need of connection to COVID internal and external services and resources.

· Cultivate collaborative relationships with partner services and agencies, including substance use disorder providers, housing organizations, supportive employment programs, and other social service agencies.

· Where/when applicable, responsible for timely and accurate documentation and reporting of event activities to increase assessment opportunities.

· Submits required documentation to supervisor within designated timeframes.

· Engages in ongoing conversation and learning with Community Health Workers and other community outreach staff in other lines of service within the organization, and partners with those staff on projects for the benefit of Alliance Care 360.

· Flexible to work evenings and Saturdays on a shared schedule if/when needed.

Preferred Qualifications:

· Be a Certified community health care specialist (worker)

· Lived experience in or close connection to a South Suburban neighborhood(s), esp. Harvey, Calumet City, Maywood, and Broadview communities.

· Experience in community education/outreach; knowledge of public health topics such as health equity, health disparities, and social determinants of health

· Ability to work with people from diverse backgrounds and experiences

· Ability to work independently, including organizing and prioritizing activities

· Ability to communicate clearly and concisely both orally and in writing

· Ability to work some nights and weekends

Job Types: Full-time, Temporary

Pay: $26.00 - $26.92 per hour
