Summary
The Transitions Clinic Peer Specialist (TCN P.S.) will provide peer support, navigation, and support services to people recently re-entering the community who are interested in establishing care at Progressive Community Health Centers (PCHC). These individuals may be impacted by a significant medical, substance use and/or behavioral health needs which can be successfully managed in PCHC's health home. The TCN Peer Specialist contributes to an interdisciplinary health care team and documents interactions in the electronic medical record. Additionally, the TCN Peer Specialist will coordinate outreach activities in the community, correctional settings, or local jails.
A Peer Specialist is a person who has lived the experience of mental health, substance use or co-occurring needs, is in recovery, has had formal training in the peer specialist model of support, and has passed the State of Wisconsin certification exam or successfully becomes certified within one year of hire. The Peer Specialist in the TCN Program would benefit from having lived experience in the department of corrections or justice system. The Peer Specialists utilize their own unique lived experience to engage the people they are serving and role model recovery during each interaction.
Responsibilities
- Provide individualized peer support and navigation through systems of care (e.g. assist with establishing a medical home, scheduling appointments, offering reminders, providing self-management support, delivering education, and assisting individuals in finding answers to questions they may have about their physical and mental health needs).
- Promote collaboration between the individual and PCHC's medical and behavioral health providers.
- Provide 1:1 peer support services to people in the community (including their home and other community-based locations).
- From a recovery perspective, provide a strength-based review of an individual's abilities, strengths, and needs.
- Provide education about the services offered at PCHC through the facilitation of group sessions in the community and correctional settings, along with community outreach.
- Conduct periodic assessments that helps identify specific needs of an individual.
- Participate as a team member in grant activities related to chronic disease and improving preventative measures.
- Work with individuals to gain insights regarding needs related to care.
- Provide referral information and support individuals accessing social services and community programs, as needed.
- Participate in community education programs, e.g., health fairs, sponsored events (Some evening and weekend work may be required).

Knowledge, Skills, and Abilities
Computer skills; accurate documentation; ability to meet deadlines; knowledge of substance use issues and mental health needs.
Working Conditions
Some of the work is done sitting at a desk and using the computer; 40 hours per week; flexible work schedule may include some evenings and weekends to meet the needs of the program. Travel in the community (including people's home) is required.
Minimal Qualifications
High school or GED/HSED required; graduate of state Certified Peer Specialist training; possess current Certified Peer Specialist certification (If such certification has not occurred by hire, state certification must occur within the first year of employment.); strong written and verbal communication skills. Personal experience with mental health and substance use needs and knowledge of recovery principles. Meet all WCS and contractor requirements for criminal and caregiver background, driver's record, and reference checks. Valid driver's license, automobile, and sufficient insurance to meet agency requirements is required.
