POSITION DESCRIPTION:
GENERAL POSITION SUMMARY
Providing in‐home personal care and daily living tasks as outlined by the child’s individual Service Plan. Provide 24-hour staff supervision and services in community-based, out-of-home care placement in a setting that is dedicated to helping at risk youth manifest high to moderate behavioral and emotional challenges, yet are capable in engaging in community-based activities successfully. In addition, our services involve a wide range of unique and intensive work with the child, family and collaboratively partnering with other local organizations. Our program consists of child and family restoration, recreational activities, educational youth group activities, independent living skills, and substance abuse awareness. We particularly emphasize on the measuring of outcomes, keeping children engaged in the community, empowering and providing service that best fits each individual's personal preferences, beliefs and goals to ensure that families are reunified and children and youths have the opportunity, encouragement and support to become responsible, productive and self-sufficient adults.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
· Monitoring and/or assisting in the daily care of consumers
· Transporting consumers to appointments and planned events
· Distributing medication to the appropriate consumer as directed
· Candidate must be involved in the day-to-day routine and documentation of Child Service Plan (CSP)
· Complete assigned paperwork for compliance and completion at end of every shift.
· Assist in preparing nutritious meals as stated on the facility menu and encouraging each consumer to participate in the preparation of meals.
· Maintain the cleanliness of the group home
· Record all unusual incidents before the end of shift
· Attending all required training, including WCH in-services and maintaining all certifications as required by WCH and DCS regulations.
· Promoting the professional standards of WCH through a friendly and productive collaboration with management, consumers, families, and the community.
· Arranges appointments for children as needed (medical, dental, psych, vision, etc.); in timely manner to ensure Health and Safety compliance.
· Writes and maintains accurate documentation (case notes, incident reports, medication schedules, client logs, facility logs, sleep patterns, client behaviors, interactions, etc.);
· Communicates timely and effectively with and other group home staff regarding the children in care at the group home
· Provides guidance, encouragement, and acts as a resource for youth in care
· Perform other duties and special projects as assigned.

QUALIFICATIONS:
· Must have a High School Diploma or GED equivalent.
· Must be a minimum of 21 years of age
· Must have valid First Aid / CPR certificates.
· Must have a valid Level 1 fingerprint clearance card.
· Must have a valid AZ Driver's license.
· Must have a clean 5-year Motor Vehicle Record
· Must have nonviolent crisis intervention training certification
· Must not have any criminal convictions within the past 10-years.
· Must not have any open or closed case with any child protective agency in any jurisdiction.
· Must have a minimum of 1 year of experience working with children.
· Meet the Health and Safety requirements (in good health, TB Test and a pre-employment physical.
· Ability to communicate in English verbally and in writing.
· Demonstrates positive role model skills and appropriate ethical behavior
· Demonstrates excellent communication skills with the ability to interact effectively and work with diverse groups to include staff, clients and family members
· Demonstrates a positive attitude, enthusiasm and energy and shows reliability in the workplace
· Demonstrates an interest and sensitivity to working around abused, abandoned and/or neglected children
