Title: Entry Level- Case ManagerDepartment: Housing and Case Management
Location: Hopelink Center Duvall, WA Initial 2-week training is in-person at Hopelink center.
Work Schedule: Full Time. Monday – Friday 8:30am – 5pm. After training, hybrid in-person with remote work options available.
Compensation: $21.00 hour

Summary: *
Hopelink provides vital resources to people experiencing financial and housing instability in our community. The Case Manager provides assessment, advocacy, referrals, and other support services to low-income families in shelter, transitional, non-time limited housing programs as well as community-based settings. The primary goal is to help families through their own resilience to make positive life changes and substantial progress towards self-sufficiency, with a focus on increasing income & obtaining and maintaining permanent housing. Currently the team is rotating days in office with client meetings taking place remotely, however some socially distant meetings take place face to face typically outdoors. This position offers paid training!

The ideal candidate will be flexible, compassionate, excited to learn, and is a team player who works well in a collaborative team environment alongside community partners. A combination of on-site and remote options is available for this role after training period is complete and job proficiency is demonstrated.

Essential Functions: *(Note: Any one position may not include all the functions nor do the listed functions include all tasks that may be found in positions of this class.)

  • Manages a moderate size case load of families.
  • Interviews and determines the needs of families and individuals in various life domains.
  • Develops case management case plans in collaboration with families and individuals.
  • Assists families and individuals with identification and achievement of goals including addressing barriers to success.
  • Responds effectively to client issues of an emergent nature.
  • Makes appropriate referrals and coordinates services with community agencies based on clients' needs and interests.
  • Ensure service interactions at all levels are done through a trauma informed perspective, Paid training for cutting edge Trauma informed approach training.
  • Practices from trauma-informed and strengths-based perspective.
  • Provides case management support to families and individuals through regular meetings. Case management support includes but is not limited to assisting families and individuals to monitor progress toward goal achievement, helping clients build and identify strengths and successes, supporting clients to develop and utilize positive problem-solving skills and helping clients identify, develop, and maintain support networks.
  • Adheres to program goals, standards of performance, policies, and procedures.
  • Provides a safe environment in which families and individuals can learn to predict the positive and negative results of their decisions and actions.
  • Provides the appropriate level of support to help clients gain and maintain self-confidence to successfully complete activities toward goal achievement.
  • Always Adheres to ethical and confidentiality standards.
  • Serves as client advocate when appropriate.
  • Makes mandated reports as required by law.
  • Maintains paper and electronic case records.
  • Completes monthly statistics and other reports as required.
  • Participates in on-going trainings, as required.
  • Actively participates in departmental and other agency wide meetings and committees as appropriate.

Desired Qualifications:

  • Experience working with diverse communities, preferably low-income, older adults, people with disabilities, immigrants, refugees, youth, and/or veterans.

Experience in social services or a community-based organization.

Computer skills required (Word, Excel, Internet). Office 365 and Teams, zoom, SharePoint, databases.

  • Introductory knowledge of human services client assessment methods, principles, and techniques.
  • Able to work in a team and independently.
  • Excellent communication skills (verbal and written).

Preferred Qualifications: *

  • Housing case management experience.
  • Bachelor’s degree or an equivalent combination of education, work experience, and/or military service.
  • Experience working with low-income families and individuals.
  • Knowledge of subsidized housing resources and housing system to address family homelessness.
  • Knowledge of financial coaching strategies and tools.

  • Must pass criminal background check through the State Patrol.
  • Valid Washington State driver’s license.
  • Reliable personal transportation.
  • Proof of personal automobile liability insurance.
