JOB SUMMARY AND SCOPE

Home visitors provide home based support services to adolescent parents and expecting or newly parenting families throughout Washington County. Services focus on supporting the family in the tasks of parenting including supporting the child’s development, establishing a positive relationship between parent and child and assisting families in accessing services for specific needs.

ESSENTIAL JOB FUNCTIONS

  • Provide home visit services and case management with identified families in accordance with PAT model and department policies.
  • Develop and maintain a trusting relationship with each family.
  • Support the family in their development of positive, healthy relationships.
  • Share information regarding infant brain development, parent/child attachment, child development, parenting techniques, child safety, support systems and community resources.
  • Work with the family to identify and address their concerns and needs.
  • Assist families in accessing appropriate community resources and referrals which may include child protective services if indicated.
  • Develop and implement individual service plan with each family.
  • Document case information such as home visit logs, community referrals, communication, and immunization records utilizing the computer database and client files.
  • Complete all necessary assessments, such as developmental screenings, home safety assessments, and reports in an accurate and timely manner.
  • Coordinate efforts with other service providers and attend appropriate team meetings at the request of the family.
  • Meet with supervisor on a weekly basis to review case work, receive support, technical assistance and direction for work with individual families.
  • Attend staff meetings and share in mutual support of all staff members and student interns.
  • Attend ongoing training when appropriate and available.
  • Complete necessary administrative paperwork in a timely manner.
  • Seeks out needed information by staying informed and involved by attending meetings, reading memos, policies, meeting minutes, newsletters, email and other appropriate information in a timely manner.
  • Reliable attendance and punctuality, including observing appropriate break times and extending a scheduled shift when necessary to meet patient demands
  • Performs duties in a safe manner, in compliance with all safety policies and procedures
  • Complies with the Code of Conduct and all appropriate policies and procedures
  • Other duties, as assigned, that are relevant to the position and department
Requirements:

MINIMUM REQUIREMENTS

  • BA and experience working with young children and families.
  • Ability to work comfortably with families in many different life circumstances.
  • A valid Maine driver’s license and reliable transportation.
  • Ability to establish and maintain honest and respectful relationships with clients-both children and adults, coworkers, supervisor, and colleagues.
  • Experience with using computers for data entry and word processing.
  • Ability to maintain strict confidentiality as stated in HIPAA and all state laws.

PHYSICAL EFFORT AND ENVIRONMENT

Moderate: Occasionally lifts up to 25 pounds independently. Occasionally pushes/pulls 100 pounds. Stands or walks often.

EXPOSURE RISK

Exposure Category III: Employee not reasonably expected to be at risk for exposure to blood borne pathogens.

QUALITY IMPROVEMENT

  • Maintains current knowledge of the hospital-wide Quality Improvement Program.
  • Collaborates with management to improve the effectiveness of the department through the Quality Improvement plans and processes.
  • Participates in the departmental and hospital-wide Quality Improvement activities as directed by the manager.

COMPLIANCE

  • Adheres to the federal, state, and local statutes and regulations.
  • Adheres to the Compliance Program policies and procedures of Down East Community.
  • Adheres to all Privacy and Security policies and procedures of Down East Community.
  • Communicates any concern related to compliance issues to manager of department or Compliance Officer.
  • Communicates any concern related to confidentiality issues to the Privacy Officer.
