Description:

  • Complete assessments, develop service plans, and connect clients to various community and government resources for support
  • Triage clients who are requesting housing assistance services
  • Identify and screen clients who are accessing services as appropriate to the level of service requested
  • Advocate on behalf of clients to decrease service gaps and remove barriers to services helping clients develop and utilize independent living skills and strategies
  • Provide information and referral to clients as requested or assessed during the course of a session. g. Ensure client confidentiality and dignity at all times
  • Verify client eligibility
  • Address mental health, housing, transportation, employment and other barriers and discrepancies that may increase risk of homelessness

Requirements: Bachelor’s degree required. Social Work, Psychology, or Public Health degree preferred.
