HOPWA Case Manager Job at The Montrose Center
Description:
- Complete assessments, develop service plans, and connect clients to various community and government resources for support
- Triage clients who are requesting housing assistance services
- Identify and screen clients who are accessing services as appropriate to the level of service requested
- Advocate on behalf of clients to decrease service gaps and remove barriers to services helping clients develop and utilize independent living skills and strategies
- Provide information and referral to clients as requested or assessed during the course of a session. g. Ensure client confidentiality and dignity at all times
- Verify client eligibility
- Address mental health, housing, transportation, employment and other barriers and discrepancies that may increase risk of homelessness
Requirements: Bachelor’s degree required. Social Work, Psychology, or Public Health degree preferred.
