Position Summary: The Hospice Aide/Homemaker is a paraprofessional member of the Interdisciplinary Team who performs various services for a patient and family as necessary to meet the patient’s personal needs and to promote comfort. The Hospice Aide/Homemaker is responsible for observing, reporting and documenting care performed and patient’s observed status.
Essential Job Functions
- Responsible for performing personal care needs after given a patient’s assignment and attending to his/her requests promptly. If unable to perform a certain task, report to primary care nurse immediately.
- Responsible for meeting the safety needs of patients and using equipment safely and properly (foot stools, side rails, oxygen, etc.).
- Responsible for giving direct personal care including complete or partial baths in the tub or shower as ordered, back rubs, oral hygiene, hair care, shaving (Using electric razor only) and changing bed linen as often as assigned.
- Responsible for assisting in the dressing and undressing of patients as assigned.
- Responsible for planning and preparing nutritious meals, including shopping, based on nutritional needs as assigned.
- Assists in the feeding of the patient as assigned.
- Responsible for providing proper care and observation of patient’s skin to prevent breakdown of tissue over bony prominence.
- Responsible for assessing and reporting on patient’s condition and significant changes to the assigned primary care nurse. Also responsible for being aware and reporting on the status changes of the caregiver or other individuals living with the patient and interpersonal issues that may come up.
- Assist in ambulation and exercise as instructed by the primary care nurse or therapist while adhering to the transfer/assist transfer policy of the agency.
- Assistance with self-administered medications as allowed by state regulations (‘reminders.”)
- Responsible providing incontinent care as needed. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs.
- Provide respite for patient’s/family/caregiver when on-site, as appropriate.
- Assists in performing range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed and demonstrated skills competency.
- Responsible for keeping patient’s living area clean and orderly, including bed linens and cloths as needed while being respectful of patient and family/caregiver environment and personal needs.
- Responsible for adhering to the organization’s documentation and care procedures and standards of personal and professional conduct.
- Other duties as assigned.
Qualifications
(Minimum qualifications will be considered required unless specifically stated otherwise)
Experience:
· Minimum of one (1) to two (2) year's experience in health care preferred.
· Experience in a hospice care preferred.
· Experience in acute care in an institutional setting preferred.
· Experience in homecare preferred.
· Understanding of the hospice philosophy, and issues of death/dying.
Education and Training:
(Acquired through formal education, outside study, training on jobs of lesser degree, or by any combination of these. May be expressed in terms of formal educational equivalents):
· High school diploma or equivalent.
· Satisfactory completion of a training program approved by M.D.P.H. for HCA-II and other regulators.
· Training equivalent to approved certification program (such as C.N.A. or Student Nurse), applicants must be oriented to home care.
License, Certification & Registration:
· Possesses and maintains current CPR Certification.
· Current Massachusetts licensure: Certified Nursing Assistant
· Valid driver’s license
· Hospice and Palliative Care Nursing Assistant Certification preferred
Other Requirements:
· Demonstrates good verbal and written communication, and organization skills.
· Strong observation skills and problem solving skills.
· Ability to read and follow written instructions and document care given.
· Ability to provide a calm and empathetic demeanor in patient’s homes.
Job Limitations:
· The Hospice Aide/Homemaker will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act. Specifically, the Hospice Aide/Homemaker will not administer medications, take physician’s orders or perform procedures requiring training, knowledge and skill of a licensed nurse, such as sterile technique.

Working Conditions:
· May be exposed to extremes of heat and cold in all weather conditions. Must drive in various weather conditions on roads in varying repair.
· May be exposed to infections and contagious diseases. Contact with patients under a wide variety of circumstances. May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations. Handles emergency or crisis situations. Travel required. OSHA exposure category:
· OSHA exposure category: Category I
§ Category I ― Position includes tasks that involve exposure to blood, body fluids, tissues.
§ Category II ―Position includes tasks that involve no exposure to blood, body fluids, tissues, but employment may require unplanned Category I tasks.
§ Category III ― Position includes tasks that involve no exposure to blood, body fluids, tissues; would not be required to perform Category I tasks.
