CareWell Services Southwest seeks a full-time Intake Coordinator for our Team.

We promote health, independence, and choice for seniors, persons with disabilities, and caregivers. As an area agency on aging, we identify seniors needs and develop plans to enable older adults to maintain their independence with dignity through person-centered services. In addition, we recognize the needs of seniors and develop strategies to help older adults to maintain their independence with dignity through person-centered services.

Position Summary:

  • Performs information and assistance to ensure requirements and standards of operations of the AAA programs provide services.

Duties/Responsibilities:

  • Completes reporting as requested for information and assistance services according to established guidelines.
  • Provides referrals and follow-up activities about appropriate community resources by established guidelines and standards.
  • Conducts initial screening functions to determine potential eligibility for Medicaid long-term care services, care management programs, community living programs, etc.
  • Provides information for scheduling participant assessments to assigned care management teams.
  • Provides backup for waitlist management.
  • Maintains records and performs data entry and recordkeeping.
  • Supports achievement of Agency mission and vision and reflects its values in work behavior.
  • Performs all other duties as assigned.

Education and Experience:

  • Associate's degree preferable in human services or related field; significant relevant experience in lieu of associate degree may be considered.
  • Alliance of Information and Referral Systems (CIRS A/D) certification in aging within 24 months of hire is required.
  • Demonstrated experience working with older persons, persons with disabilities, and their caregivers in various settings desirable.
