JOB PURPOSE AND REPORTING STRUCTURE: Under the supervision of the Director of Pharmacy, the Lead Clinical Pharmacist is a member of the clinical interdisciplinary care team. The Lead Clinical Pharmacist delivers pharmaceutical care in accordance with the policies and procedures of Outreach Community Health Centers.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Ensure that medications reach the patient in the correct dose, form, vehicle and at the correct time. This includes tablets, liquids, drops, capsules, ointments, injections, inhalers, creams, beads, liposomes, continuous delivery systems and other forms that medications may take.

2. Take responsibility for the accurate, appropriate and timely use of medications administered to patients by staff in the clinic setting.

3. Take responsibility for the accurate, appropriate and timely dispensing of medications for patients upon presentation of valid prescriptions in the outpatient pharmacy setting.

4. Supervise and monitor the work of pharmacy technicians in compliance with State and Federal Requirements.

5. Teach and respond to queries of patients and their family's providers and other staff regarding accurate, appropriate and timely use of medications including indications, dosing regimens and side effects.

6. Stay current on new information and developments and educational material in Pharmacy to serve patients, providers and other staff.

7. Collaborate with providers and other health care staff including Community Service, Behavioral Health, Primary Care and other Medical Specialties to deliver safe, effective and economic medication use and delivery.

8. Counsel patients on dosage, administration, intended effects and unintended side effects of medications.

9. Monitor the stages of medication therapy to improve reporting of side effects and challenges with overall safe delivery.

10. Give special consideration to patients requiring multiple medications, complex regimens of therapy, and barriers to compliance with the goals of therapy.

11. Participate and complete special assignments and projects as assigned by the Director of Pharmacy.

12. Assists in inventory control and proper storage of medication.

13. Maintain professional standing and licensure in accordance with the State of Wisconsin Department of Safety and Professional Services.

14. Maintain technical competency and skills required for employment as a pharmacist.

15. Complete required documentation on a timely basis.

16. Comply with all Federal, State and local statutes that address discrimination.

17. Comply with safety rules and regulations of the organization.

18. Maintain the Pharmacy consistent with organizational standards.

19. Learn the ordering, billing and receiving system presently in use at the pharmacy.

20. Master the 340B Federal Drug Pricing Program.

21. Other job-related duties as may be assigned.

SUPERVISORY RESPONSIBILITIES:
The Lead Clinical Pharmacist will carry out these responsibilities in accordance with state and federal laws and regulations applicable to pharmacy. This includes participating in the interviewing, training and evaluating of staff and other assignments such as resolving problems and addressing complaints under the leadership of the Director of Pharmacy.

The Lead Clinical Pharmacist must be able to perform all essential duties and meet all obligations of a clinical pharmacist. EDUCATION AND EXPERIENCE:
The Lead Clinical Pharmacist must be a graduate of an accredited School of Pharmacy and licensed to practice as a pharmacist without restriction in the State of Wisconsin. Knowledge of the laws and regulations of the State of Wisconsin and the Federal government is required. Work experience as a pharmacist will be assessed.
