Jamestown Family Health Clinic is searching for a full-time Licensed Clinical Social Worker (LCSW), to join our growing Behavioral Health team!

The LCSW will consider social determinants of health to develop comprehensive care plans in treating complex medical patients in an outpatient setting. This position works closely with Family Medicine physicians and advanced care practitioners to assist in the diagnosis and treatment plans for behavioral health (BH) conditions. The LCSW credentialed BH counselor will work directly with patients in one to one BH counseling visits to provide long-term goals focused on improving quality of life, incorporating treatment for behavioral health diagnoses, and offering non-pharmacological and complementary options for symptom management. Our clinic population leans heavily towards the treatment of older adults but includes adolescents, teens, and younger adults.

As a Licensed Clinical Social Worker, you will:

  • Provide age and culturally appropriate comprehensive bio-psychosocial assessments for mental health, substance abuse, and medical needs in the primary care setting.
  • Provide behavioral health services to individuals, couples, and families, including, but not limited to. counseling, intake, assessment/evaluation, diagnosing, treatment planning, crisis intervention and referred to CD treatment
  • Provide liaison services, social work support, and referrals to local resources and community agencies, including medication assistance programs.
  • Provide psychoeducational information as appropriate for patients and/or family members or accompanying caregivers.
  • Collaborate with specialty care services (i.e. BH, MAT, Chronic Disease Management) to develop and facilitate the group visit model.
  • Collaborate with Quality Improvement teams to develop and implement processes for data collection.
  • Provide consultation and feedback to primary care providers.
  • Provide training to JFHC staff on specific behavioral health issues (e.g. depression, mindfulness, health lifestyle, choices, anxiety, and care giver challenges).
  • Develop and maintain familiarity with MH/SA related psychopharmacology.
  • Remain current and knowledgeable about local and regional resources for mental health and substance abuse treatment, developing collaborative relationships where possible.
  • Remain current on basic funding rules and guidelines as it relates to behavioral health integrated care delivery of services.
  • Participate in outreach educational activities in the community as assigned.
  • Maintain a regular schedule in clinic in collaboration with the treatment teams at each clinic that best accommodates their need.
  • Maintain all necessary documentation and participate in ongoing dialogue about content, quality, and organization of BH progress notes, self-management goals, care plans, etc.
  • Participate in all JFHC meetings and training as directed or assigned.
  • Work collaboratively with colleagues in primary and specialty care teams.
  • Assist in identifying, developing, and integrating new BH treatment opportunities for MH/SA patients when possible.

What are the Required Qualifications for this position?

  • Masters degree in social work and three (3) years post licensure experience
  • Must possess knowledge of counseling techniques, crisis intervention, and have the ability to make independent judgment related to counseling as a direct service is essential.
  • Experience and skills in group facilitation and planning processes
  • Must maintain relevant Washington state Professional License (LCSW)
  • Read, analyze and interpret professional periodicals and journals, technical procedures and government regulations.
  • Excellent organizational, coordination, and management skills with experience working in simultaneous, often diverse programs to effectively manage time, resources and client caseload.
  • Analyze, evaluate and diagnose situations and adopt effective plans of action.
  • Assess physical and mental health concerns.
  • Demonstrates strong computer skills, including proficiency in Microsoft Suites, knowledge of data management (EMR) and analysis when needed.
  • Exceptional interpersonal skills including effective verbal and written communication with demonstrated willingness to work within a team environment.
  • Knowledge of and the ability to apply relevant laws and regulations as they pertain to social services and specific professional licensure.
  • Knowledge of Indian culture and communities.
  • Ability to travel locally and regionally as assigned
  • Must maintain a valid Washington state drivers license with insurance
  • Must be able to successfully pass criminal background investigation
