St. Ann Rehabilitation and Nursing Center, in Dover, NH, is a beautifully renovated 54 bed facility offering short- and long-term care in a warm and supportive atmosphere where residents are cared for according to their individual needs.
St. Ann is looking for a full time (40 hours/week) Minimum Data Set (MDS) Nurse to join our Nursing Administration Team. This position works under the supervision of the Director of Nursing Services to ensure RAI guidelines are followed with a focus on person-centered care. The ideal candidate will have a background in long term care nursing setting.
Job responsibilities include:
Knowledge of all aspects of the RAI/MDS process to include person-centered care planning, Medicare PPS reimbursement, scheduling, month-end triple check, appeals process and Medicare certification process- Provide ongoing education and training for MDS and person-centered care planning
- Understand and follow through with the RAI process, including MDS assessment, Triggers and Raps, and Care Planning.
- Verify completion and accuracy of MDS prior to submission.
- Knowledge of New Hampshire specific case mix reimbursement preferred
- Review of case mix on a quarterly basis to ensure accuracy
- Review of Medicare assessment scheduling on a monthly basis including unscheduled OMBRA assessments
- Maintain the Medicare certification/ recertification binder to meet the requirements of Medicare physician certification for Part A & B services.
- Collaborate with Rehab Services/DNS/Administrator to determine optimal time frame to gather MDS data, and set the ARD within the appropriate PPS window.
- Maintain current PPS/Medicare and Medicaid/Private assessment schedules with proper reference dates for regular distribution to therapies, dietary, social service, activities and nursing.
- Demonstrate current knowledge of nursing practices and procedures as they pertain to Long Term Care including state, federal, and OBRA regulations.

Must be fully vaccinated against COVID-19 and have received first dose prior to starting- Registered Nurse (RN) with a valid New Hampshire license
- RAC-CT certification preferred
- Minimum 2 years’ experience in long term care, including experience with the RAI process and person-centered care planning
- Must have strong clinical and communication skills
- Experience with the RAI process and reimbursement requirements for both federal and state
- Must be highly organized with the ability to multi-task throughout the day
- Experience with Microsoft PowerPoint, Word, Excel and Outlook preferred
- Experience in developing templates to utilize in facility for audit/educational use
