Our MDS COORDINATOR has a MONDAY - FRIDAY schedule, facilitating the timely and accurate completion of both the RAI and Care Management process. This is comprehensive, from admission to discharge in accordance with Samaritas policies / state and federal guidelines, ensuring our AWESOME residents are provided with the highest quality care.

Some Duties and Responsibilities

  • Collaborate with the Interdisciplinary Team to assess the needs of the resident.
  • Coordinate and/or conduct accurate and timely resident assessments according to state and federal regulations.
  • Obtain the highest coding accuracy level of Minimum Data Sets (MDS), ensuring timeliness, with information obtained from a medical record review, observation and interviews with the Community’s staff, residents, and family members.
  • Maintain MDS schedule tracking systems to ensure adherence to timeframes and due dates. Review MDS data for accuracy, submit to state and federal regulatory agencies and follow-up with action required.
  • Maintain Resident Assessment Instrument (RAI), updating diagnosis codes and allergies.
  • Participate in Quality Assurance Performance Improvement (QAPI) initiatives including Quality Improvement checks and month-end audits. Utilize regulatory reports to identify areas of opportunity for improvement.
  • Conduct initial and on-going training to staff as it relates to MDS and resident’s Activities of Daily Living (ACL) documentation.

Job Qualifications

Education, Training, and Licensure/Certification

  • Current/un-restricted State of Michigan RN license

Experience

  • One year experience in long-term care required; MDS experience
