Please note that individual total compensation for this position will be determined at the Company's sole discretion and the wage range for this role considers a wide range of factors including but not limited to skill sets; experience and training; licensure and certifications; and other business and organizational needs. At SNAHC, it is not typical for an individual to be hired at or near the top of the range for their role and compensation decisions are dependent on the facts and circumstances of each case. A reasonable estimate of the current range is $35,360.00-$47,840.00
Position Summary:
This Clinical Care Assistant (CCA) will collaborate with medical staff and other departments to improve health outcomes of patients through panel management and care coordination. The main responsibilities of this position include assisting in completing required grant activities, assisting patients in accessing all levels of care, improving the quality of chronic care services, assisting medical care providers in chronic disease management, and participating in continuous quality improvement. Attends monthly medical insurances meetings and ensures that the Gaps in Care reports are being completed and submitted.
Essential Functions:
- Perform routine clinical tasks.
- Utilizes I2I and other tools to identify and manage high-risk, complex-need and other persistently high utilization patient populations.
- Assist primary care providers in formulating processes to identify and address unmet care needs. These processes might include PDSA cycles that include outreach, medical interventions and follow-up evaluations.
- Daily outreach and follow up with patients via phone, letters, member portal messaging and face-to-face interaction in order to coordinate patient care, implement action plans and communicate PCP recommendations to patients.
- Maintain an accurate patient registry via bi-monthly outreach and lost-to-care follow up in order to provide current and accurate data.
- Promotes Team-Based Healthcare approach through ongoing support of core principles.
Monitor tasks in NextGen and complete within 48 hours.
Maintain an awareness of the cultural and spiritual needs of the American Indian/Alaskan Native population.
Assists in providing education as appropriate.
Coordinates Health Maintenance Day events with the medical clinic leadership in order to promote and complete quality measures.
Maintain the highest respect for confidentiality in order to maintain patient privacy and sensitivity.
Abide by industry best practices (e.g. proper PPE, ergonomic posture, etc.) to maintain a safe working environment; report safety issues or concerns to the Medical Clinic Manager or the Safety Officer immediately; review Safety and Health plans, understand, and ask questions regarding safe practices in your work area.

Minimum Qualifications:
- 1-year experience working in a health care setting
- 1-year experience in customer service
- Medical Assistant certificate from an accredited institution.
Preferred Qualifications:
- Experience with Nextgen or other electronic health records systems
- Experience conducting patient outreach to assist in the coordination of healthcare appointments and services.
