Job Summary and Qualifications
POSITION SUMMARY:
The Neuroscience Nurse Navigator acts as a consultant, facilitator, and educator to the neuroscience population and is responsible for assisting the patient through the care delivery system. Responsibilities include: education of the patient, coordination of all aspects of care, advocacy on behalf of the patient, communication with the multi-disciplinary team and with the patient and family when appropriate. Oversite of the Brain Tumor Certification program and metrics. Oversite of the Focused Ultrasound Program with navigation and growth initiatives.
SPECIFIC ELEMENTS AND ESSENTIAL FUNCTIONS
- Demonstrates the knowledge, skill, and coordination to provide nursing care guidance to the patient from screening to surgery and beyond. Systemically and continually performs the function of assessing, planning, implementing and evaluation the care according to the nursing process.
- Perform initial screening and triage via phone and make appropriate referrals as defined. Complete screening questionnaire and others forms as necessary for intake.
- Provides a central point of access for the patient. Serves as an essential link between the patient and all other care providers for the patient’s preferred treatment plan.
- Educates the patient on the diagnosis and treatment options helping to make the care seamless, continuous and comprehensive. Initiates and documents patient teaching including family and significant others based on assessment of needs.
- Participates in unit rounding specific to patient population
- Coordinates treatment and scheduling. Facilitates appointments and consults and support services within established service standards.
- Responsible for Joint Commission Brain Tumor Certification reviews
- Evaluates program metrics and makes recommendations for care
- Navigates Focused Ultrasound patients and participates in program outreach and growth initiatives
- Communicates with referring Physicians, team physicians, interdisciplinary team and any other necessary personnel.
- Facilitates long term follow-up and tracking of patient outcomes.
- Offers insurance advocacy by providing the necessary patient navigation through the continuing of care. Provides necessary clinical information required for pre-authorization and validates that pre-authorization is complete.
- Functions in an organized and time conscious manner. The navigator partners with the patient, interdisciplinary team to provide well-coordinated, timely, compassionate, exemplary, interdisciplinary care. The navigator communicates with all members of the care team, as appropriate about the patient needs and concerns.
- Collaborates with Program Coordinator or Facility Designee for outcomes measures including Length of Stay, Re-admissions, HCAPS, and core measures.
What qualifications you will need:
EXPERIENCE
Required - Three years of experience in an adult neurosurgical setting with a basic knowledge of the care of neurological patient
Preferred - Experience in case management
EDUCATION
Required - Graduate of an accredited school of nursing.
Preferred - Bachelors of Science in Nursing
LICENSURE
Required - Licensed in the State of Virginia as a registered nurse
