Position Summary:
The primary duties of the Emergency Shelter Crisis Direct Care Staff are to carry out rehabilitation and support functions such as hands on care and training to intellectual and developmentally disabled residents as specified in the resident’s individual or positive behavioral support plans. This includes but is not limited to support, training, and consultation to families, and provides behavioral interventions under the clinical supervisor, registered nurse and/or resident manager's direction. Additional duties include collaboration with a multidisciplinary treatment team to modify maladaptive behaviors of the resident, processing utilization, admission and discharge paperwork and preparing reports.
Position Qualifications:
- Minimum Education: Have a minimum of a high school diploma or high school equivalency and completion of twelve (12) credits or four (4) classes of college coursework in social work, human services, human development, psychology, or a closely related field
- Minimum Field-of-Expertise: Have one (1) year experience in the provision of psychological, social, or personal assistance services in a human service delivery setting that includes interviewing consumers and/or observing and recording behaviors for use in the development and implementation of treatment, or positive behavioral plans.
- Required Certification/Registration: Basic Life Support: (current & renewed prior to expiration o CPR and First Aid o MANDT and/or CPI o PBS and Person-Centered Planning
- Preferred Experience: Work experience with adults or children with developmental and/or intellectual disabilities, dual diagnosis, medical conditions, including mental and physical disabilities.
Duties and Responsibilities:
- Be a member of the PBS/IP team and participate in treatment planning for participants
- Participate in developing, writing, implementing, evaluating, and revising overall treatment and / or housing goals and plans in conjunction with the treatment team
- Participate in comprehensive assessment of psychiatric history, mental status, and diagnosis; physical/dental health; use of drugs or alcohol; education/employment; social development functioning; activities of daily living; and family/legal guardian structure and relationships
- Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process
- Document participant progress towards treatment goals and maintain a permanent record of participant's daily activities according to established methods and procedures
- Participate in daily staff organizational meetings and treatment plan review meetings
- Participate in facilitating various Positive Behavioral Support Plan goals and objectives for participants
- Provide ongoing assessment and response to treatment
- Perform job coaching, problem solving, and support on and off the job site
- Aids with nutritional education/food preparation, admission and discharges, transportation options
- Ensure participation in the completion of shift change duties.
- Must have a compassionate and supportive attitude that aligns with CARE Hawaii’s mission and vision.
- Ability to establish rapport with care providers and Individuals in a state of crisis.
- Ability to accurately document and log CMO events after completion
- Responsibly reports to work on time/consistent attendance and follows program protocol when calling in sick and or for emergencies.
- Must be able to coordinate with other agencies, such as police or emergency personnel including follow-up with CM/Circle of Support.
- Must be able to attend all supervisions, trainings, meetings, etc.

Required Skills: Mature, reliable, and responsible. Must have clear, concise, and empathetic communication skills. Must be culturally sensitive and aware. Must know the basics of ethics, HIPPA standards, and problem solving. Must display sound judgment, a good work ethic, and personal integrity.
Technical Skills: Basic use of a computer and/or tablet, Microsoft Word Suite, fax machine, telephone, and text and chat features on the telephone or computer.
Working Environment: Indoor/outdoor, open windows or air-conditioned environment; require some travel, over-time, weekend, and holiday work, working with other Federal and/or State accounting bodies, noise level in the environment may be moderate to noisy. Exposure to infectious diseases is a relatively common risk.
Special Demands: Must be able to work as the ID/DD emergency shelter on a scheduled shift. Ability to respond effectively to the varied needs and demands of multidisciplinary team members. Have access to helpful, adequate, competent, and continuous supports and services. Knowledge of how to recognize and act upon a life- or-death situation.
Consumers Served: CARE Hawaii ID/DD will provide services to any adult or adolescent individuals with an intellectual/developmental disability who meet the criteria of the program. Ability to interact with people who are escalated, emotional, anxious, and angry
Physical Demands:
1. Sit/stand for long periods of time, use of proper ergonomics, the ability to focus on a computer and/or tablet for long periods of time.
2. Ability to assist with the high volume of calls; ability to communicate effectively both verbally and in writing is essential.
3. Ability to follow instructions and motivation to refer to various rehabilitative groups. Frequent reading of printed materials is essential.
4. Listening skills necessary to effectively respond to consumers.
5. Ability to identify and support the daily interactions and to deal with each situation in a stress-free manner.
Additional Duties
1. Type letters, memos, and reports
2. Interfaces with CARE Hawaii, Inc. staff
3. Participates in conflict resolution as necessary
4. Assess workload and appropriately change work and break times if necessary
5. Willingly chart filing, ensuring charts are properly completed thoroughly by case manager and with QMHP signatures and monitor charts to ensure charts are always audit ready

Other Duties
Assist with Quality Improvement development and monitoring
Participate in team meetings
Prepare written or electronic reports as requested
Willingly perform other duties/responsibilities as requested
