Provide professional coordinated patient care service as a member of the primary care team following established standards and practices. Perform routine tasks necessary for efficient patient care and assist providers as needed.

Duties include but are not limited to:

· Coordinate and maintain HIV program policy and procedures

· Educate staff on policy and procedure for HIV program.

· Monitor HIV program quality measures.

· Coordinate HIV Outreach programs and provide community education.

· Monitor adherence to HIV policies and procedures.

· Administers prescribed medications and treatments in accordance with nursing standards.

· Dispenses medication under provider’s supervision.

· Participates in nursing care essential to the assessment, promotion, maintenance, and restoration of patients’ health and wellness.

· Prepares equipment and aids physician during treatment, examination, procedures, and testing of patients.

· Observes, records, and reports patients’ condition and reaction to drugs and treatments to physicians and nurse manager.

· Maintains timely flow of patients.

· Greets patients and prepares them for physician examination.

· Screens patients for appropriate information.

· Arranges for patient testing.

· Performs laboratory procedures when necessary.

· Maintains and reviews patient records, charts, and other pertinent information.

· Posts tests and examination results.

· Make provider aware of abnormal tests and examination results.

· Maintains exam rooms for necessary supplies and materials.

· Prepares list of medical supplies needed.

· Gives guidance and counsel to other clinic staff as needed.

· Orients and educates patients and their families by meeting them; explaining the role of the patient care coordinator; initiating the care plan; providing educational information in conjunction with direct care providers related to treatments, procedures, medications, and continuing care requirements.

· Carries out interdisciplinary care plan as outlined by rendering provider. Utilizes case management tools by participating in meetings; coordinating information and care requirements with other care providers; resolving issues that could affect smooth care progression.

· Establishes a comprehensive plan of care to assist in reaching patient milestones working directly with care teams, patients and support system.

· Monitors care plans for progress toward desired care outcomes and works with direct care to expedite scheduling and completion of tests, procedures, and consults

· Participates with interdisciplinary team to evaluate outcomes and intervention effectiveness. Implements team recommendations.

· Handles confidential matters concerning clinic policies and practices in accordance with HIPAA regulations, maintaining strictest confidentiality.

· Maintains infection control within the center.

· Performs related work as required and/or assigned.
