Overview:
The Program Manager for Health and Wellness Centers will primarily serve the Del Valle and Southeast Health and Wellness Centers
and will serve as the on-site manager for health and wellness activities, events, and supporting programs. In this role, they build and
maintain innovative programs to help connect services to the community in a valuable way. They serve as primary point of contact
for outreach activities in designated areas of operation, manages staff implementing community outreach projects and builds
relationships with operations teams across the Enterprise; develops processes to track,manage and report on outreach programs,
activities and initiatives. This position will oversee scheduling, activity coordination, community events, and volunteer management.
This position will maintain relationships to attract activities and programs to the centers that support the health and wellness of area
residents and will work closely with the Sr. Director of Community Health and Wellness Initiatives to ensure that the center programs
and activities meet the needs and requirements of Central Health and affiliated entities, such as CommUnityCare and Sendero.

This position is considered Hybrid, which means that individuals in this position may work both at an approved Offsite location and

Onsite at a primary location or multiple locations based on Business Needs.

Qualifications (Knowledge/Skills/Abilities):

Knowledge of:
o Health care coordination, delivery and needs for people with low income
o Network of health care providers in Austin / Travis County
o Techniques used in community organizing, volunteer coordination, or community engagement
o Effective social media techniques and strategies
o Mac OS X and Microsoft Windows, Microsoft Office, Excel, PowerPoint
o List management and distribution software
o Familiarity with design software such as Adobe InDesign, Illustrator, Photoshop preferred
o Designing content for and measuring outcomes of Twitter, Facebook, website, Constant Contact
o Website management
o Social media platforms including Facebook, Twitter and Instagram

Skilled in:
o Strong content creation and writing skills with the ability to present information in a straightforward manner
o Experience in community organizing, volunteer management, and/or public engagement
o Developing briefing materials and collateral material
o Creating and delivering engaging presentations
o Creating online community and dialogue via Twitter, Facebook, website, e-newsletters
o Experience promoting events
o Experience in photography and / or videography preferred
o Bilingual English/Spanish language capability strongly preferred
o Measuring outcomes of communications
o Experience running a grassroots policy campaign

  • Work closely with CommUnityCare Practice Administrator and team to coordinate and manage use of Center facility space
  • Maintains collaborative working relationships with existing community partners to expand coordinated efforts and reach
target and additional populations
  • Develop and attract health and wellness activities for benefit of patient population and residents
  • Serve as a community liaison in coordination with Central Health Community Engagement Team
  • Work with Central Health and affiliated entity staff to monitor effectiveness of health and wellness programs and their
alignment with strategic and operational objectives
  • Represent Central Health by participating in community coalitions and engagement actitives
  • Actively participate with and act as a resource for community groups, internal staff, and outside agencies
  • Hires, coaches and trains direct reports.
  • Manages staff and vendors responsible for helping residents and their families navigate and access health coverage and
health care services, identify and overcome barriers to care, and provide health education
  • Manages multiple staff providing project updates to community members in corresponding areas
  • Oversees multiple staff tracking activities and reports on productivity and project milestones
  • Manages the development of new initiatives and grassroots efforts to inform the community of health programs, new and
existing services
  • Manages the production of event logistics, the inventory of promotional items and educational materials
  • Plans outreach events and organizes teams both internal and external
  • Photographs and documents activities during community events and assists with creating content for social media
  • Proactively researches and recommends opportunities for new partnerships and initiatives

People Management:
  • Supervises assigned administrative supervisor onsite
  • Manages Central Health outreach staff and outreach vendors
  • Recruits, trains, and develops outreach staff.
  • Manages project updates to community members and presents to leadership about key milestones,
accomplishments and strategies.
  • Manages activities and reporting by staff and vendors regarding project milestones as they support
Central Health Strategic Priorities.
  • Manages the development of new outreach initiatives and helps refine/evolve Central Health outreach
strategies.
  • Creates and manages implementation of community outreach programs, activities, and initiatives.
  • Identifies, develops, and manages meaningful relationships with community partners.
Qualifications:
MINIMUM EDUCATION: Bachelor’s Degree in communications, community relations, public relations, or a related field.

MINIMUM EXPERIENCE: 7 years work experience in healthcare/community clinic operations and/or social work, community
relations, health and wellness-related programming and working with diverse populations. Bilingual (Spanish/English) preferred.

PREFERRED EXPERIENCE: 10 years of directly related experience. 2 years of supervisory experience.
