The Project Coordinator, Care Pathway Development for the Care Pathway Initiative reports to the Dir, Bundled Care. This role is essential in the building and implementation of healthcare, evidence-based Care Pathways across multiple Care Transformation Services (CTS) and across the entire HMH network. The major CTS for Care Pathways include Cardiovascular, Behavioral Health, Neurosciences, Orthopedics, Pediatrics, and Oncology. The goals and outcomes associated with this position directly support improvement of patient care, healthcare delivery, and patient outcomes. The requirements associated with the role of project coordinator include planning, organizing materials and information, scheduling and leading meetings, and collaborating directly with network healthcare providers at all levels of the organization. This position is responsible for working with clinical subject matter experts to identify the care that is provided across the continuum of care within a CTS, document information in written and chart form, store that information in a methodical manner using Google tools, and meet required timelines. The Project Coordinator, Healthcare Pathways for Care Pathways will also be responsible for abstracting medical data directly from patient medical records (both electronic and paper) and deriving information from that data.

Responsibilties:
A day in the life of a
Project Coordinator, Care Pathway Development at Hackensack Meridian Health includes:
Develops and maintains effective and efficient project plans, tracks project progress, and prepares and presents progress reports for project stakeholders, including the Care Pathway Team, CTS Leadership, physicians, and clinicians across the network.- Engages with multiple, multidisciplinary subject matter experts by planning meetings in advance, preparing agendas, and following desired identified meeting outcomes/goals.
- Conducts and facilitates formal and informal, cross-functional meetings with experienced clinical subject matter experts such as physicians, nurses, and administrative leaders to foster collaboration for the development and delivery of network Care Pathways.
- Partners with clinical subject matter experts to identify, discuss, and corroborate delivery of care.
- Participates in the determination of new treatment plans by documenting potential improved treatment algorithms.
- Conducts medical record data abstractions for quality assurance as required.
- Identifies and reports internal and external departmental process and procedural changes and recommends improvements as needed.
- Stores and maintains information and project deliverables utilizing assigned electronic file formats and in assigned locations.
- Assembles and manipulates data utilizing spreadsheets and other forms of information storage.
- Develops professional and trust-based team relationships.
- Collaborates with and supports other project coordinators as needed to meet all assigned team goals.
- Meets all assigned individual, team, and departmental goals.
- Abides by and maintains HIPAA requirements and protects PHI.
- Other duties and/or projects as assigned. Adheres to HMH Organizational competencies and standards of behavior.
Qualifications:
Education, Knowledge, Skills and Abilities Required:
- Bachelor's degree.
- Minimum of 2 years of medical/nursing/ clinical experience.
- Strong knowledge in healthcare, medical terminology, medical records, and professional communications.
- Demonstrated ability to handle complex work and achieve high productive output.
- Flexible, self-motivated, and detail oriented.
- Ability to effectively work on multiple projects concurrently.
- Effective communication and coordination skills are essential.
- Ability to organize priorities.
- Experience with process mapping.
- Ability to pivot quickly and effectively between competing priorities and tasks.
- Ability to work independently and in group/team settings.
- Travel may be required between network hospitals.
Education, Knowledge, Skills and Abilities Preferred:
- Advanced degree.
- Minimum of 4 years of experience in the medical field as a clinician (physician, nurse, nurse practitioner, etc.).
- Knowledge and user experience with EPIC EHR.
- Knowledgeable and proficient with Google tools.
- Data analytics.
- Experience with use of Lucidchart or Visio.
- Knowledge and skills with process improvement using, one or more of, Lean, Six Sigma, PDCA, etc.
