Job Summary

The RN Case Manager conducts initial admission assessments and identifies patients who do not meet criteria and ensures patients receive the most appropriate level of care. The RN Case Manager actively participates in daily rounds to coordinate care in conjunction with other members of the healthcare team and provides and facilitates optimal health care/discharge planning.

Required Qualifications

• Associates Degree in Nursing if actively pursuing BSN or bachelor’s degree in Business, Health Care Administration or equivalent with bachelor’s completion required within 3 years of hire date
• Two years of experience as RN
• Current KY RN licensure or compact license with privileges to work in Kentucky
• BLS required from American Heart Association

Preferred Qualifications

• Bachelor’s Degree in nursing or an RN with a bachelor’s degree in Business, Health Care Administration or equivalent
• One year experience in discharge planning/case management experience in an acute care facility
• Case Management Certification (ACM, ANCC-Nurse Case Manager or CCM)

Pay: $1,265.00 - $1,580.00 per week
