As an RN Case Manager with Quality Private Duty Care, you will assess, plan, implement, and coordinate all case management activities with your assigned patient group and guide the patient(s) in defining their health goals and taking appropriate steps to achieve their optimal state of wellness. The RN Case Manager educates on many topics including: disease process, treatment understanding, and medication role. The role utilizes a collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet an individual’s health care needs.

We are seeking a Case Manager for patients in the Lenoir City, Loudon & Maryville areas.

Some of the things you will do:

• Create plan for care across the continuum, integrating patient/family preferences and values.
• Monitor patient care through assessments, evaluations and/or patient records.
• Advocate for resources and removal of barriers.
• Maintain ongoing dialog with supervisor, appropriate staff and other health providers to ensure effective implementation of health plan.
• Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.
• Promoting and monitoring effective utilization of health care resources.
• Assuming a leadership role with the interdisciplinary care team.
• Facilitating the care of patients by managing, coordinating and monitoring resources to achieve optimal clinical outcomes, utilization, service and financial outcomes.
• Assessing and facilitating appropriate hospital admissions using evidence based criteria.
• Managing the collaborative process of assessment, planning, facilitation and advocacy for options to meet an individual's health needs.

Requirements of the RN Case Manager

• Current and unencumbered License from the state
• Current CPR certification
• Valid Driver’s License & reliable transportation
• Auto insurance
• Must be able to pass a criminal background and drug test
