Job Title: POH/ORS Intake Coordinator

GENERAL PURPOSE OF JOB:

Create, manage & streamline a process of Intake for all potential Pathway of Hope and Operation Red Shield clients. Begin necessary Intake documentation and gathering of necessary forms. In cases of financial assistance, the Intake Coordinator will work with Operation Red Shield financial assistance funds in conjunction with outside partner agencies’ funds to meet the financial needs of the client(s). The Intake Coordinator will work with the client(s) to complete the necessary financial application(s) and gather all necessary supporting documentation. *This position is grant funded and may not continue if the grant(s) are not renewed.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • First point of contact for ORS/POH program.
  • Manage ORS/POH referral line and email.
  • Develop professional relationships and connections with community agencies and veterans programs so that outside resources can meet client needs when possible and appropriate.
  • Respond to requests for service from area providers and initiate ORS/POH intake process.
  • Manage the financial assistance process for ORS clients by distributing and gathering necessary applications, documentation, and vendor W-9s. Submit a complete financial assistance application to the appropriate personnel for approval and payment.
  • Provide veteran-specific training to case managers.
  • Assign clients to case managers as necessary.
  • Review client cases to ensure compliance with goal setting and program completion every 90 days.
  • Ensure follow-up of clients after exiting the program every 90 days for one year.
  • Manage database of ORS/POH statistics.
  • Perform outreach activities as necessary.
  • Perform administrative functions as needed.
  • Participate in training for Operation Red Shield and social services as directed.
  • Work in coordination with local administrators and staff.
  • Work cooperatively with the Regional Coordinator, who oversees implementation and works to maintain the integrity of the initiative Division-wide.
  • In all areas, always provide service without discrimination.
  • Familiarity with TSA policies and procedures and able to maintain confidentiality and ethical conduct in accordance with organizational policy and carry out business in a manner consistent with the mission, values, and policies of the organization.

SPECIAL SKILLS, CERTIFICATES, LICENSES, AND REGISTRATIONS. List any special skills, licenses, certificates, or registrations that are required to perform the essential duties of this job.

  • Associate in social work, Human Services, or related field and 3-5 years of case management experience; bachelor’s degree in social work or related field preferred.
  • Computer proficiency
  • Ability to communicate effectively, both verbally and in writing
  • Excellent interpersonal skills
  • Excellent listening skills, including empathetic listening.
  • Experience with Veteran Services or Military Service
  • Must have an interest and ability to work with people and clients of diverse racial, ethnic, and socio-economic backgrounds in a sensitive and culturally appropriate manner.
  • Ability to work without close supervision on assigned duties and willingness to seek and accept supervision and feedback as needed.
  • Ability to maintain confidentiality.
  • Meet The Salvation Army KeepSAfe guidelines.
  • Possess a valid State Driver’s License and meet TSA driver’s guidelines.

Job Type: Part-time

Pay: $22.00 - $27.00 per hour
