What You'll Do:

Located in Iredell county, this vital role promotes the whole person, integrated health care through evaluation, assessment, care planning, coordinating, tracking, and monitoring outcomes for individuals served. The focus will be on closing gaps in care for physical and mental health, social determinants of health, and other non-health-related needs. Tailored Care Manager may also serve members in Surry and Yadkin counties.

Additional requirements include:

• Bachelor's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human service area. Master's level degree preferred.
• Minimum of two years of experience working directly with individuals with behavioral health conditions.
• Valid driver's license.
