Spiritual Care Coordinator Job at Fair Acres Geriatric Center

Position Summary:
This person is an experienced Chaplain, with background in theology, chaplaincy or pastoral care, and is responsible to assess the spiritual needs of each resident and to provide skilled spiritual support and care.
As a chaplain the coordinator offers spiritual care to all residents according to their needs, serving the sick, the dying and their families through personal presence and prayer.
Responsibilities:
Spiritual Care to Residents:
- Do a Spiritual Assessment on every new resident to ascertain the specific needs of the resident.
- Document each Spiritual Assessment.
- Respond to all referrals from doctors, nurses, social workers and hospice personnel.
- Offer spiritual support and prayerful presence to residents who are dying and to their families and create spiritual materials for those interested in praying with residents and their families.
Church Services
- Coordinate all religious, spiritual, and church services for the residents.
- The coordinator plans a monthly calendar of spiritual services.
- Coordinate The Anointing of the Sick.
- Document each resident who has received the Anointing of the Sick.
- Plan and set up music for Catholic Masses.
- Coordinate religious experiences for different faiths: e.g., Seder.
- Coordinate Ash Wednesday and the Distribution of Ashes for residents and staff.
- Coordinate activities with Therapeutic Activities Services (TAS) and oversee communication among TAS workers and Spiritual Care volunteers on the floors (e.g., Bible study, rosary, fellowship groups).
- Be a contact and offer assistance to Hospice Staff developing programs for employees and family members.
Volunteers:
- Recruit and train “friendly visitors” who are assigned one unit or specific residents to visit, usually once a week.
- Recruit volunteers who help “gather” residents for Mass and other religious Services and return them to their rooms after Services.
- Recruit Christian Ministers to provide Church Services, Communion.
- Plan and provide continuing In-Services and Support Service for Volunteers.
Memorial Services:
- Organize and plan an annual Memorial Service.
- Offer Bereavement Services as needed
Requirements:
Bachelor’s Degree, progressive experience in pastoral care or chaplaincy required.
Master’s Degree in Divinity or pastoral counseling preferred.
Experience working with the sick, death and dying.
Strong teamwork skills.
