Who We Are

One Cross Medical Clinic is a Rural Health Clinic (RHC) striving to serve the underserved in our community. We are a faith-based clinic that has been in operation since January 2015, being housed in our new facility since January 2021. Our goal is to provide excellent holistic healthcare of mind, body, and spirit, serving our patients as our own family because we believe that Jesus is the Healer.

We currently have a team of providers, peru support specialists, a full-time x-ray technician, and a full lab housed in our facility who all work alongside our wonderful office team to provide the very best patient care to our neighbors. Generalized Job Summary

A Case Manager, or Social Service Assistant, is responsible for helping those with chronic illness, mental health issues or life-altering conditions to gain access to rehabilitation programs and other community services. Their duties include meeting with clients and their families, connecting clients with professional services and maintaining consistent communication with clients to assess their wellbeing and make changes to their treatment plan as needed.

What You Will Do

As a Targeted Case Manager for One Cross Medical Clinic, you will have a special opportunity to provide, coordinate and direct the provision of mental and behavioral healthcare in a faith-based facility focused on holistic care of our patient’s mind, body, and spirit. You will closely collaborate with the FNP’s, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent behavioral health care.

-Assess and reassess all physical, environmental and emotional factors to determine behavioral health needs for patients in the clinic setting

-Works in cooperation with the Peer Support Team in providing high-quality healthcare to your patient base

-Utilize a combination of agency resources and nationally recognized standards of practice to achieve excellent care

-Coordinate the total Plan of Care and maintain continuity of patient care by liaising with appropriate staff and the physician

-Actively participates in both departmental and interdepartmental meetings and trainings

-Provide supervision and evaluation of Peer Support Specialists

Qualifications Required

-Bachelor’s Degree

-Minimum of one (1) year of experience as a registered nurse

-Current CPR certification

-Valid Driver’s License and reliable transportation

-Previous Case Management experience

Salary: Up to $45,000.00 annually
