Position Description:
Transition Support Specialists provide support to individuals being discharged from Institutes for Mental
Disease (IMD) into community based independent living situations. Specialists assist individuals in identifying
the services in their communities that will assist them in living independently. In-reach is an engagement,
education and support effort designed to accurately and fully inform adults who have a serious mental illness
(SMI) or a serious and persistent mental illness (SPMI) about community-based mental health services and
supported housing options including but not limited to; the availability of tenancy support services and rental
assistance. Services range from identifying locations of appropriate service providers to teaching individuals
community skills needed to be successful in living as part of a community.
Responsibilities:
- Accesses eligible individual’s interest in supported housing
- Fully explains the benefits and financial aspects of clinically appropriate community based integrated
- Facilitates and accompanies individuals on site visits to permanent housing with tenancy rights
- Explores and addresses the concerns of any of ACH residents who decline the opportunity to move to
supported housing or who are ambivalent about moving to supported housing, despite being qualified
for such housing
- Reviewing residents’ housing preferences

- Provides linkages to ongoing peer support services as a step in education/exposure to the housing
- Educates individuals about services covered under the North Carolina State Plan for Medical Assistance,
- Helps individuals who are transitioning from IMD facilities to find resources they need, while become
familiar with the program. Tasks may include identifying local resources, identifying methods to obtain
basic support services, locating local merchants for shopping, local pharmacies, local methods of
transportation.
- Uses the Community Integration Plan as a guide – support individuals to achieve the goals of the plan
- Supports/ assist participants in locating local support groups, linking with community resources
- Tracks individual progress, following up with support services to assure time lines and needs are being
- Assists individuals in developing linkages with community resources and local provider agencies.
- Works with Transition Coordinator to track placements and conduct follow up of residents placed into
- Responsible for conducting follow up for each transitioning individual for a minimum of 3 months, or as
the individual plan indicates.

Knowledge/Skills/Abilities:
A high level of diplomacy and discretion is required to effectively negotiate and resolve issues with minimal
assistance from management. This will require exceptional interpersonal skills, highly effective communication
ability, and the ability to use good judgment to make prompt independent decisions based upon relevant facts.
Problem solving and conflict resolution skills are essential. Ability to build trust and establish rapport with
transitioning individual, family members, facility administration and community resources.
As a Transition Support Specialist, the employee must exhibit an understanding of agency business and have
knowledge of community resources and services available to Eastpointe enrollees. The ability to establish and
maintain favorable working relationships with consumers, families, stakeholders and Eastpointe employees is
necessary. The ability to express oneself clearly and effectively in oral and written form as well as knowledge
of cultural awareness and differences, and the ability to work with/or serve a diverse population is also needed.
Basic knowledge of: the social, economic, and environmental conditions of recip ients served; gross
characteristics of behavior of mental illness; and available community resources. Ability to relate to clientele in
a supportive and therapeutic manner; to follow written and verbal directions; to record basic data and report
findings verbally to supervisor; to exercise judgment in appraising home situations and to report findings to
supervisor; and to represent agency effectively in community.
Education/Experience/Licensure:
NC Certified Peer Support Specialist with a minimum of high school/GED with a minimum of 2 years’
experience working with the MH/SU population in a community setting.
