Symphony of Lincoln Park is now hiring a (Transitional Care Nurse / Psychotropic Nurse) to join our dynamic team from Monday to Friday, 12 noon to 8 pm!

Transitional Care Nurse / Psychotropic Nurse Responsibilities:

• Manage and direct the transition of care throughout the health care continuum
• Coordinate care and services for a high acuity population
• Communicate status and care plan with physicians, referral sources, patients, families and facility staff
• Collaborate and coordinate with referral sources
• Work collaboratively with physicians and other members of the health care team
• Collaborate on implementation of specific clinical programs as they pertain to the patient population
• Identify and report on specific outcomes and clinical metrics within the population being managed
• Facilitate safe discharge through appropriate discharge planning

Specific Educational/Vocational Requirements:

• Registered Nurse
• Case managment experience

Preferred:

• BSN or MSN preferred
• Experience with transitions of care or discharge planning
• Experience with high acuity population
• Either Acute Care or Skilled Nursing experience

Job/Technical Skills:

• Written and oral communication skills
• Ability to work effectively in an interdisciplinary team
• Ability to effectively meet the needs of both external and internal customers, such as managed care and accountable care organizations, families, facility staff
