JOB TITLE: Service Coordinator
REPORTS TO: LCSW, Director of Adult Services
DEPARTMENT: Suffolk County Department of Health Services, Division of Community Mental Hygiene Services
SITE: William J Lindsay County Complex, 725 Veterans’ Memorial Highway, Building C 016, Hauppauge, NY 11788
HOURS: Monday-Friday, 9am to 5pm
QUALIFICATIONS: Bachelor’s Degree required, Master’s Degree preferred
SALARY RANGE: $50,000-$53,000, dependent on credentials and experience
JOB DESCRIPTION:
The position is an administrative position in the Suffolk County Care Coordination program, a Specialty Mental Health Care Management Program, designated by Office of Mental Health (OMH) to work with Health Home (HH) PLUS and Assisted Outpatient Treatment (AOT) clients. The range of population served may vary, including individuals diagnosed with significant behavioral disorders, such as severe mental illness and substance use disorders. Work is performed under the supervision of the Director of Adult Services of Suffolk County Division of Community Mental Hygiene.

Pay: $50,000.00 - $53,000.00 per year
