POSITION TITLE: Community Health Worker

RESPONSIBLE TO: Behavioral Health Program Manager and Clinic Practice Manager

FLSA STATUS: Non-Exempt

Purpose of this Position: The Community Health Worker (CHW) assists patients in navigating community resources and healthcare systems while advocating for underserved individuals. The CHW will create connections between communities and healthcare systems by managing care and care transitions for vulnerable populations. Community Health Workers provide culturally appropriate services to assist the healthcare team with health education, chronic disease management, and preventative services. CHWs assist with insurance enrollment and complete required federal and/or state consumer assistance training to provide fair, accurate, and impartial assistance. Additionally, the CHW represents Valley Family Health Care at community outreach events and identifies new collaborative efforts and partnerships to advance outreach goals. Requires extensive traveling within the service area from Emmett, Idaho to Vale, Oregon

Requirements:
QUALIFICATIONS:
1. High School diploma or equivalent.

2. Strong computer skills, including proficiency with web-based applications.

3. Fundamental understanding of health outreach and social service programs.

4. Self-starter with strong organizational skills and ability to multi-task.

5. Experience in working with low income/minority populations.

6. Bilingual in English/Spanish preferred.

7. Excellent interpersonal communication and writing skills.

8. Ability to work as a team and independently depending on the project.

9. Must have a current drivers’ license and clean driving record.

10. Must pass a multi-state criminal background check.

RESPONSIBILITIES:
1. Providing facilitation between communities, health systems, and social service systems.

2. Providing culturally appropriate and accessible health education and information.

3. Assuring that patients &/or designated care giver get needed services.

4. Providing clinical and social supports.

5. Advocating for individual and community needs.

6. Administering selected screening procedures (Social Determinants of Health, PRAPARE).

7. Locating resources for improved health outcomes.

8. Provide comprehensive and culturally sensitive information about Idaho and Oregon’s Health Insurance Exchange and public insurance programs and benefits.

9. Assist applicants in completing and submitting applications.

10. Conduct follow-up with individuals and families on the progress of goals and troubleshoot barriers.

11. Track provided eligibility assistance and successful enrollments in QHP, Medicaid and CHIP insurance plans.

12. Ensure the confidentiality of all applications, records, and information.

13. Serve as a point of contact for individuals and families contacted during off-site outreach activities regarding health insurance enrollment.

14. Support outreach and enrollment efforts by working collaboratively with community partners—a. school nurses, public health programs, churches, labor groups and other community entities.

15. Participate in outreach community events as needed.

16. Comply with and successfully complete federal and state consumer assistance training required for personnel carrying out consumer assistance functions.

17. Participate in ongoing trainings, learning sessions, conference calls, webinars and other professional development opportunities.

18. Document and maintain records of encounters with individuals and/or families.

19. Timely completion of referrals.

20. Assist with meeting quality measures by completing tasks related to VFHC initiatives.

21. Perform other duties as assigned.

Physical Requirements:
1. Ability to lift up to 25 lbs.

2. Prolonged sitting, bending, stooping, walking and standing.

3. Correctable vision and hearing.

4. Must be able to read, write and speak clearly.

5. Manual dexterity.
