POSITION SUMMARY:

To establish a medical care home and provide outreach in the target community to patients who may be lost to a lack of follow-up. Participate in various outreach activities to educate and inform the assembly of the availability and scope of ancillary health care programs offered at Milwaukee Health Services Inc. Maintain a patient caseload commensurate with daily job duties and responsibilities.

POSITION-SPECIFIC COMPETENCIES/ESSENTIAL FUNCTIONS/DUTIES & RESPONSIBILITIES:

  • Assume patient assignments and maintain a patient caseload professionally and effectively.
  • Assist patients with making follow-up appointments.
  • Use sensitivity in communicating with patients regarding ongoing needs and patient assessment.
  • Comply with federal, state, and local regulations regarding patient confidentiality and HIPAA.
  • Become knowledgeable of the social services network in and about the City and County of Milwaukee.
  • Complete patient needs assessment through in-depth conversation to determine referrals and services required.
  • Maintain a calendar of events, notifying the manager before to ensure proper staff coverage.
  • Assist patients with referrals internally to the department’s Social Workers and other ancillary health care departments and externally to referral specialists and other programs and services in the community.
  • Organize and distribute health education and health prevention materials, e.g., pamphlets and brochures, at various community events (e.g., at health fairs, MHSI patient waiting for areas/lobbies, in various MHSI departments) as appropriate.
  • Participate monthly in data collection, record-keeping, data entry for their assigned caseload, and other Department activities.
  • Participate in ongoing training programs for family support workers and attends additional continuing education activities as appropriate.
  • Attend departmental, team, and other meetings related to Social Work and as required by MHSI Staff.
  • Knowledgeable of the National Association of Social Workers (NASW) Code of Ethics.
  • Advocate for patients within the community health center system and link patients with community-based resources.
  • Perform other duties as assigned.

POSITION REQUIREMENTS:

  • Education: At a minimum, high school graduate or equivalent. College coursework and post-secondary school training or education are highly preferred.
  • Experience: Experience in a community-based agency or community health organization is preferred.
  • Expertise: Ability to communicate and interact with individuals and families from diverse ethnic, cultural, and socioeconomic backgrounds. Ability to identify and maintain confidential information with integrity. Ability to effectively work independently and as a member of the team. Ability to effectively communicate with individuals as well as with groups. Requires basic computer skills, e.g., Microsoft Office Software. Excellent organizational skills and knowledge to problem-solve. Ability to prioritize tasks and work assignments.
  • Language: Comprehend and use primary language, either written or spoken, to communicate information and ideas.
  • Hours of Work: Must be able to work flexible hours. It may vary based on Organizational needs.
  • Travel: May vary based on Organizational need.

Job Type: Full-time

Pay: $18.00 - $20.00 per hour
