As a Geriatric Care Manager, you will provide a range of direct, personal and therapeutic support to client(s) in a community-based setting while managing the coordination between Care Teams. This position is also responsible for overseeing the various health care aspects on medication management, nutrition, injury/disease process, and self-care. .

We're looking for a qualified and compassionate Care Manager who is capable of working independently and works well with other medical professionals. The ideal candidate will have previous experience working in geriatrics and will be capable of overseeing the well-being of our patients. This position is emotionally demanding, but offers many rewards including the opportunity to make a positive impact on the lives of elderly individuals and their families.
Responsibilities:
- Communicate with medical professionals.
- Develop a plan of care.
- Determine the needs of the patient and plan care accordingly.
- Create a care team to provide support to patient and family.
- Prescribe medications and treatments for patients.
- Monitor patient's treatment plan.
- Assess patient's response to treatments.
- Gather information about the patient's social and financial situation.
Job Type: Full-time
Pay: $75,000.00 - $90,000.00 per year
