GENERAL DESCRIPTION
The LPN will be able to work in a recovery-oriented setting with an understanding of the overall system of care, promote principles of recovery, and advocate for de-stigmatizing those struggling with use of substances. The LPN will assist with initial screening/medical assessment to participants seeking buprenorphine and buprenorphine-naloxone treatment through Amara Wellness Services Office-based Opioid Treatment (OBOT) program. LPN will ensure medical orders are carried according to OBOT Provider, monitoring participants’ progress and response to care over time; participating in treatment team meetings and case discussions; maintaining timely and accurate chart records, providing relevant education for those in care, communicating effectively with other health care providers in the community. LPN will provide intervention in crisis management situations, assisting in distributing Narcan sample supplies for overdose reversal, and providing training on its use.

DETAILED JOB DESCRIPTION
Responsibilities:
- Facilitate and participate in the initial screening and physical examination
- Ability to use evidence-based screen tools as needed and required
- Participates in ongoing assessment for signs and symptoms of over-sedation or withdrawal and alerts OBOT Medical Provider and Clinical Manager
- Participate in intake as directed by OBOT Physician or medical staff. This may include, but is not limited to measuring vital signs, obtaining medical/psychiatric histories, performing a review of systems, collecting urine/blood specimens as needed and performing TB tests
- Log all prescription and OTC medications being prescribed to participant
- Ensure all necessary regulatory reports are generated at required intervals
- Enter all medical orders and information related to the prescribing into MAT dosing system
- Assist with telephone orders as delegated/from OBOT Medical Provider
- Ensure lab results are received and available in the electronic record. Notify OBOT provider and substance use counselors of positive participant drug screens
- Participate in coordination of dosing with community detox programs, inpatient facilities, detention facilities and services for homebound participants
- Participates in coordinating guest dosing as ordered
- Identify abnormal client behavior, document and report to RN or MAT medical provider
- Participates in ongoing client education related to existing treatment plan
- Assist RN or medical provider with management of medical crises
- Participate in weekly treatment team as delegated by RN
- Complete any additional tasks as delegated by RN
- Support Psychiatric, Primary Care and Residential programs as directed
- Ability to identify and report client sedation
- Ability to draw blood / obtain urine specimens / measure and record vital signs
- Following the outlined in the NC Division of Medical Assistance Clinical Coverage Policy: 1A-41 for OBOT services.
- Completing all other assigned clinical-administrative tasks within the scope of practice.
ADMINISTRATIVE RESPONSIBILITIES
- Attend and actively participate in Team Meetings
- Attend and actively participate in individual supervisions
- MUST be able to submit progress notes within 24 to 48 hours of date of service in participant’s permanent record
- Submit clinical forms, paperwork, and report(s) according to deadlines
- Submit all and other assigned paperwork according to deadlines
TECHNICAL SKILLS
- Basic computer skills for using Microsoft Word
- Ability to use Electronic Health Record system
- Writing skills that include sentence structure, grammar, and use of clinical terminology

WORK EXPERIENCE REQUIREMENTS
- 2 or more years' experience working in inpatient or outpatient setting with individuals that have substance abuse and / or mental health problems
- Basic knowledge about severe and persistent mental illness and substance use disorders
- Works within your scope of practice according to credential, training, and skills
- Understand the various levels of care based on the continuum of services
- Flexible and willing to work in a “non-traditional” work environment
EDUCATION/CREDENTIAL REQUIREMENTS
- Completion of accredited Licensed Practical Nurse program
- Unrestricted LPN Licensure in NC
- 2 or more years' experience working in inpatient or outpatient setting with individuals that have substance abuse and / or mental health problems
- Completes and maintains all required trainings set forth by NC Division of Medical Assistance Clinical Coverage Policy: 1A-6 which is specific to Office-based Opioid Treatment and Amara Wellness Services, Inc.
