Job Description:
Job Summary:
Administers, directs, and coordinates inter- and intra-departmental activities. Work is performed in accordance with Kentucky regulations for Registered Nurses, established hospital standards, nursing department/unit structure, process and outcome standards, regulatory and accreditation requirements, and the philosophy, goals and objectives of the hospital, nursing and education services.
Essential Duties and Responsibilities include the following. Other duties may be assigned.

Administers Dialysis appropriate medications and necessary blood or blood products while on dialysis machine

Education of Patient and Family
9.
Attends all required safety training programs and can describe their responsibilities related to general safety, department/service safety, specific job related hazards, and national patient safety goals.

Provides complete verbal report with appropriate information. Effectively communicates to other care providers/team member’s necessary information. Delegate tasks based on patient need and staff competency, and assures tasks are completed. Responds appropriately to customer requests

Performs ongoing physical assessment for purpose of evaluation of patient and patient’s response to care.
3.
Formulates/Implements Nursing Plan of Care. Selects and implements nursing interventions based on assessment and nursing diagnosis. Develops mutual goals with patient and family.
4.
Makes appropriate decisions utilizing evidence based practice. Recognizes changes in patient’s physical and emotional condition and institutes interventions. Makes decisions in crisis situations based on assessment and prompt intervention. Establishes realistic workflow priorities delegating appropriate tasks to assure work is completed within allotted time.
5.
Supervises the proper care, use and repair of supplies, equipment and charges. Disposes of soiled linen in appropriate container. Keeps work area and department clean and neat

Performs professional responsibilities . Acts as a professional role model at all times. Functions as mentor for new employees. Makes decisions and adapts to unit changes precipitated by increased workloads.
7.
Performs and reinforces formal and informal patient teaching, utilizing available resources. Reinforces patient teaching, including teaching done by physician and other departments.
8.
Communicates with others in completing work assignments and accomplishing department/unit goals. Utilizes appropriate channels of communication
9.
Keeps physician thoroughly informed of any sudden changes in patient’s condition or insidious changes requiring medical intervention. Completes reports with appropriate information
10.
Assess patient interactions that may impact patient care. Recognizes pertinent and individual patient needs. Reevaluates dialysis plan on an ongoing basis, based on progress towards goal and patient outcomes.
11.
Accountable for accuracy and completion of all documentation requirements. Documents all care given, including patient preparation and patient response to treatment. Documents physical/emotional care rendered with patient response and/or outcome. Provides effective care in a code or crisis situation. Acts as a preceptor when applicable, identifying and following through with performance or knowledge deficits
12.
Demonstrates ability to address staff/peers with concerns related to performance expectations and behaviors that may effect work environment, image, patient care and customer service. Requests and accepts constructive criticism
13.
Demonstrates fiscal responsibility through appropriate use of resources. Adheres to Nursing Department dress code. Attends mandatory in-services and unit meetings

Minimum Education, Experience, Training, and Licensures Required:
Graduate from an accredited School of Nursing required, Bachelor of Science in nursing preferred. Current provisional license as a Registered Nurse by the Kentucky Board of Nursing or compact state license. BCLS required. EXPERIENCE: No experience required for part time and full time employees.
